Header Errors loading Plugin Settings Page

This is what I get on top of the page when going into the settings page for the plugin.  The rest of the page's formatting is also screwed up.  The plugin was installed directly through WP.

(xxx is used instead of the full website path for security)

Please let me know if it's solved with version 3.3 of the plugin.

It now says you do not have permission to access this page when you click on settings, also settings is missing from the admin menu.

The plugin doesn't have a option called Settings.

There are the following options to click on:
AVH F.D.A.S
  Overview
  General Options
  3rd Party Options
  IP Cache Log
  F.A.Q

Does the menu AVH F.D.A.S show up?

Hello,

I had the same problem. And: Yes it has a option Settings - i get the Error "You do not have sufficient permissions to access this page".

An other Menu is not available.
