Custom permalink not working - activating AVH

My permalink isn't working for the category pages when I click on a category from within the AVH Widget.  Would someone be able to tell me why and how I can correct it? 

For e.g. if I click on a category in the breadcrumbs I get: http://melanieotg.ca/blog-kids-activities/outdoor-playgrounds-splash-pads/

but if I click on Outdoor Playgrounds & Splash Pads from within the widget, I get: http://melanieotg.ca/?cat=84
